A hallucination is when a language model states something confidently that is not actually true.
Because a model generates plausible-sounding language, it can produce a wrong fact, a made-up detail, or an incorrect figure with the same confidence as a correct one. This is a core reason installed systems keep a human approval step on anything customer-facing, to catch exactly this kind of error before it goes out.
